How do I get my W2 from Dignity Health? ›

If you are an active CommonSpirit Health employee, please access MyBenefits by logging into the MyBenefits site. If you have questions, please, contact the CommonSpirit Health Benefits Contact Center at 855.475. 4747, Option 1 (Dignity Health) or 855.475.

Otherwise, you'll need to contact your employer or SSA for a copy. The quickest way to obtain a copy of your current year Form W-2 is through your employer. Your employer first submits Form W-2 to SSA; after SSA processes it, they transmit the federal tax information to the IRS.

Employer contributions to your Health Savings Account are reported on Form W-2 Wage and Tax Statement, Box 12 with a code "W." An amount entered here will flow to Line 7, Column C of California Schedule CA.

If you're an online banking user, your tax documents will be available to you on the Member Website. If you chose to get paper tax documents, you'll get a copy in the mail. Where can I find my tax documents on the Member Website? Get your 1099-SA and 5498-SA forms in the Message Center or from the Resources page.

Reporting on the Form W-2

Employers that are subject to this requirement should report the value of the health care coverage in Box 12 of the Form W-2 PDF, with Code DD to identify the amount. There is no reporting on the Form W-3 of the total of these amounts for all the employer's employees.

Box 10 of your W-2 shows the total amount of dependent care benefits that your employer paid to you or incurred on your behalf.
